Make sure you have read and understood which forms are required and that the correct procedures are followed filling them out. Please make sure you send the form in with your booking form or bring the correct forms with you on the day completed correctly.

Tandem Medical Forms

There are TWO DIFFERENT TANDEM MEDICAL forms.

EVERYONE doing a tandem skydive has to fill out ONE of these medical forms.

PLEASE NOTE – It is our policy not to allow anyone who uses or relies on a wheelchair to undertake a tandem skydive. There is no upper age limit for the tandem skydives, however if you are over the age of 60 then we will require you to attend the centre for a pre-assessment before accepting a booking

If you rely upon walking aids for your mobility then we will require you to attend the centre for a pre-assessment before accepting a booking.

Please read carefully the TANDEM STUDENT PARACHUTIST MEDICAL INFORMATION AND DECLARATION (form 115A) to see which one you need to fill out.

If you have not suffered in the past or do not currently suffer from any of the conditions listed on the TANDEM STUDENT PARACHUTIST MEDICAL INFORMATION AND DECLARATION (form 115A) you may fill in this form yourself without going to the doctor.
If you have had in the past or do currently suffer from any of the conditions listed you must get your doctor to sign & stamp the OTHER form called the – STUDENT TANDEM PARACHUTIST REQUEST FOR MEDICAL ADVICE (FORM 115B)
A LETTER FROM YOUR DOCTOR IS NOT ACCEPTABLE. THE DOCTOR MUST SIGN & STAMP THE STUDENT TANDEM PARACHUTIST REQUEST FOR MEDICAL ADVICE FORM.

BPA Membership Agreement Form

The BPA Membership Agreement (also known as the under 18 parental consent form).

This is for 16 and 17 year olds as we do require written parental/guardian consent. Both the tandem student and the parent/guardian are required to sign this form.

Solo Student Medical Forms

EVERYONE has to fill out a Solo Student Medical Form. There are two different Medical Forms.

Form 115C Solo Student Parachutist Self- Declaration of Fitness to Parachute.

Form 115D Solo Student Parachutist Request For Medical Advice.

You need to read Form 115C first .

If nothing listed on Form 115C applies to you then you can fill in Form 115C yourself. If you are 16/17 years old a parent/guardian will need to witness this form.

If something listed on Form 115C applies to you then you must take the other Medical Form 115D to your Doctor to have it signed and stamped by them.

A letter from your doctor is NOT acceptable.
